The Lutheran Home Earns CMS 4-Star Quality Rating

Belle Plaine, MN (December 29, 2020) —The Lutheran Home: Belle Plaine, a skilled nursing facility, earns Centers for Medicare & Medicaid Services’ (CMS) four-star overall quality rating with five stars in staffing and RNs (registered nurses) staffing. The overall quality rating is based on ratings for annual health inspections, quality measures, and staffing.

CMS uses a complex rating for skilled nursing facilities nationwide. This metric was developed to help individuals, their families, and caregivers compare nursing homes easily. CMS updates its report quarterly.

“Our staffing model allows for adjustments depending on our residents’ care needs,” said Angie Mott, RN and Medicare Manager at The Lutheran Home: Belle Plaine. “It’s an asset to have more registered nurses available in our care community.”

The Lutheran Home: Belle Plaine continuously monitors quality as a care team that includes the medical director and pharmacist. The team takes a broad look at their processes, systems, and residents’ characteristics. Focusing on consistency for stays by residents, the team narrowed in on decreasing hospital and emergency room visits. They accomplished their goal by:

  • Increasing infection control measures, which are critical during COVID-19
  • Educating team members with follow-up and hands-on training available
  • Scheduling routine provider visits in addition to drop-in visits to address changes in residents’ characteristics
  • Initiating higher-level treatments on a short-term basis through a partnership with OPTUM
  • Adjusting the admissions process to adapt to residents’ care needs
  • Discharging residents to their home or a less restrictive setting through a partnership with Pro-Rehab
