Social Services Designee

Our Care Community is seeking a full-time (20 hours/week) Social Services Designee who is compassionate, caring, and can maintain a strong connection with team members, families, and residents. The Lutheran Home: Belle Plaine campus takes great pride in providing quality care to our residents, developing relationships that embraces care of the soul, working with our sister facilities to enhance continuity of care and relationships. Our newest Social Services Designee must have the skillset to manage multiple tasks, a desire to grow and learn, and manage unpredictable situations effectively while maintaining compassion, professionalism, and creating solutions.

Responsibilities and duties:

• Evaluate and assess the cognitive, psychosocial, emotional, and well-being of the residents
• Admission and discharge planning, care planning, and care conferences experience preferred
• Provide support to residents and families
• Participate in collaborative committees, quality improvement programs, and team meetings
• Perform administrative requirements, such as completing forms or reports, as needed
• Complete tasks assigned by Social Services Director.

Qualifications and skills:

• Experience in Healthcare, Social Work, Community or Behavior Support Services and/or other related field or 2 year degree in related field
• Preferred minimum two years of long term care experience (but not necessary)
• Good critical thinking and problem solving skills
• Exhibit ability to prioritize responsibilities
• Flexible and able to work under stressful situations
• Good verbal and written communication skills
• Self-directed and work autonomously
• $19.53 – $24.11 / hour

The Lutheran Home: Belle Plaine is a 60-bed skilled nursing facility that comprises of a Transitional Care Unit (TCU), Long-Term Care (LTC) and memory care.
